Trinity Hospice and Palliative Care - Living Well Service

Our Living Well service helps patients to achieve personal goals after they have been diagnosed with a life-limiting illness.

The service supports patients through tailored clinics and group sessions to live as well as possible and achieve ambitions after their diagnosis. For example, getting back to work following treatment, improving fitness or cognitive abilities or just some wellbeing advice.
