NHS - Wyre Adult Mental Health Community Mental Health Team

The Wyre CMHT is based at Mountcroft Resource Centre in Fleetwood.

We are a multi disciplinary team who work with adults between the ages of 16 to 65 years who have severe and enduring mental health issues.

The team works alongside people to support them through a period of mental ill health with the aim of promoting recovery they also work with people in need of longer term management of mental ill health or relapse by working together on recovery focused support in self care management and individual care planning.
