Integrated Adult Learning Disability Team

The team is made up of Health and Social Care Professionals from Blackpool Council, NHS Blackpool and Lancashire Care Foundation Trust who work together to provide a service to adults with a learning disability.

What do we do?

• Joint health and social care assessments of an individuals needs.
• Identify services and support systems to meet individual needs.
• Develop care and support plans alongside the person and other relevant people.
• Complete specialist assessment of complex health, emotional, psychological and social needs and development of care/support plans
• Provide specialist consultation and advice to other health, social care and provider agencies to promote equal access to all available services for people with a learning disability.
