Fylde Coast Community Mental Health Team (CMHT) provide a service for older adults, usually over the age of 65, who need assessment and treatment of mental health disorders which due to their severity or complexity cannot or have not responded successfully to treatment in Improving Access to Psychological Therapy (IAPT) services or primary care.

Referrals are accepted from the Initial Response Service (IRS) or internal referrals from other services such as The Memory Assessment Service (MAS), Inpatients or Rapid Intervention and Treatment Team (RITT).

We also accept direct referrals for anyone who is under Section 117 aftercare review.

The service provides specialist assessment and treatment, and high-quality support for older people with complex mental health and social care needs, including people living with dementia. The key focus will be one of therapeutic optimism and service users will be assisted to maximise their quality of life by promoting recovery, social inclusion and living well with dementia and other significant mental health conditions. The service will also provide assessment and specialist support to carers and families.

The team consists of Community Mental Health Nurses, Occupational Therapists, Psychologists, Medics and Community Support Workers and the team work closely with RITT, MAS and Social Services.

Fylde Coast Older Adult Community Mental Health Team are open Monday to Friday, 9am to 5pm
