Fylde Coast Memory Assessment Service

The Fylde Coast Memory Assessment service supports patients living across the Fylde Coast.

Our aim is to provide good quality, early diagnosis and intervention for all patients without a diagnosis of dementia of any age,  whatever point they are on the dementia journey.

Referrals are accepted directly from GP’s and patients are offered a telephone clinical triage by a clinician in the Memory Service.

Patients will be offered further appointments to discuss diagnosis, treatment options and post-diagnostic support.
