The Special Educational Needs and Disabilities (SEND) Code of Practice 2014 states that the government’s “vision for children with special needs and disabilities is the same as for all children and young people – that they achieve well in their early years, at school and in college, and lead happy and fulfilled lives”. This includes preparing young people for:
- Higher Education and/or Employment
- Independent Living
- Participating in Society
The support for preparing for adulthood needs to start early, in Blackpool we start this process in Year 9 (aged 14) and it should centre around the child or young person's own aspirations, interests and needs. A Transitions Co-ordinator attends Year 9 Educational, Health and Care plan Annual Reviews to give information, advice and guidance to the young person and family. It is intended to provide clarity as to the respective roles of Children’s and Adult Social Services in managing the transition process and to:
- Improve continuity of care between adult and children’s teams.
- Give young people and their families as much information as possible about choices available in adulthood.
- Give clarity about responsibilities during the transition process.
- Provide a structure to try and ensure a seamless move between services, and a framework for identifying and dealing with gaps as part of that move.
Higher education and/or employment
There are different educational and employment options:
Local education provision options are at Blackpool and Fylde College, Myerscough College and Beaumont College Blackpool. Through our Blackpool Young People Service (BYPS), support for preparing for work such as CV writing, help with interview techniques, short courses to prepare for employment are available.
Project Search is also available for supported employment opportunities. Project Search is an unpaid, Further Education programme, focussing on employability skills and three work placements. Coaches support individuals throughout the programme. When you do get a job you or your employer are able to apple for an Access to Work Grant. This will enable reasonable adjustments in the workplace to be made.
Blackpool Council encourage and support young people moving into adulthood with opportunities for independent living. This depends on a number of criteria and individual's needs. Your Transitions Co-ordinator will provide you with further information and advice, which is why it is important to engage with this service during your Annual Reviews.
Participating in society
During their time at school, a young person may have already gained access to social support and direct payments to help with participating in social and leisure activities in the community, indepent of parents, carers and family.
During their transition to adulthood, those opportunities continue and there are various adult provisions available such as gardening projects, helping with running a cafe, bowling, walking and various social opportunities including provisions for young people with sensory needs. Again, your Transitions Co-ordinator will advise you of all these developing opportunities. Your Transitions Co-ordinator is a key person to engage with during your annual reviews.
Post 16 Transition Co-ordinator
We provide transition support for young people in receipt of learning disability and social care support. We help young people in special schools prepare for adulthood through guidance, support and informal learning.
We carry out Section 139a assessments to plan the Post 16 learning of young people with SEND in mainstream schools. Contact Angela Davies - Personal Adviser 01253 477915
During Year 9 Blackpool Council screen and assess for services that meet the needs of the young person. This might result in engaging with one of the following teams at the age of 18: Autism Specialist Team, Learning Disability Team, Adult Social Care Team
Your Transition Co-ordinator is key to this process and will provide information, advice and guidance during this process.
