Paediatric Outreach Team

The Paediatric Outreach team is made up of experienced and trained nurses, their job is to help you and your family manage at home if you have a chronic disease like Asthma, Cystic Fibrosis, constipation etc., they aim to help you to visit hospital as little as possible.

The team support children and young people from 0 - 18 Years.

The team supports and trains you and your family to take care of your needs at home, for instance learning about Injections, home oxygen, and enteral feeds.

The outreach team also works with other services such as Brian House to provide special care such as Oncology and Palliative care.
