Age UK Lancashire - Hospital Aftercare Service Fylde and Wyre Residents

We can offer help to adults who have had a hospital stay. We can also support adults during a period of illness in the community to avoid admissions.

We offer two types of service.

• The take home and settle service will provide a time limited service for up to 3 hours direct support that includes transport home from hospital and offers low level support with the intention of helping the client settle back into their home. This can also include the aftercare service which is a follow up service at home. Clients will be given information on home safety, aids to daily living, medication issues, care needs, support networks, home hazards, fire safety, finance issues, benefits advice and carer support. 
  
  
• Practical help can be offered in the form of shopping, cleaning, taking clients out for confidence building.
